Important! If you enrol in a Commonwealth supported place or apply for a FEE-HELP loan you must have obtained and read a copy of the relevant booklet. Your provider will supply you with a copy of the booklet.

Forms

Commonwealth assistance forms are only available from your provider. They are:

Request for Commonwealth support and HECS-HELP,
Request for FEE-HELP assistance,
Request for FEE-HELP assistance - Open Universities Australia, and
OS-HELP debt confirmation.

Your provider may also allow you to complete the form electronically when you enrol and they can advise you of whether they offer this facility.

These forms are produced in triplicate. One copy is for the Tax Office and the second is held by your provider. You should retain the third copy for your records.
